Bluestern Family Medical Records

For patients of Amanda Steventon, MD or Tessa Rohrberg, MD, you can request your medical records to be sent directly to another health care provider by following these steps:

​

1.  Fill out the request form and fax, email or mail it in.
 

2.  Submit $25 payment

​

Please allow 2-3 weeks for processing.

A $25 fee will be charged for all Medical Record Requests.  If your records are less than 10 pages, you will be charged a lower rate.  See details here.
